When you’re looking for a new physician, you’re bound to have questions. To help make things a little easier, we’ve provided answers to some of the basic questions people have. If there’s anything else you’d like to know, we encourage you to call us.

How do I make an appointment?

Select the location and physician you’d like to see, and call the appropriate number. If you need immediate care, let us know and you will be promptly scheduled. Our goal is to provide timely medical treatment.

How do I reach the different EMPCA offices?

When you call the number at the location you’re being seen at, a receptionist will ask for your name, telephone number and the nature of your call. You may also be asked to provide your birth date, any known allergies and current insurance information. If your call is not urgent, your physician will return your call as soon as possible or after office hours.

What if I have to cancel my appointment?

We understand that things come up and that you sometimes have to cancel your appointment. All we ask is that you notify us at least 24 hours in advance. When you call, give the name of your physician and the date and time of your appointment. You may reschedule your appointment at that time.

How do I get a copy of my medical records?

When you request a copy of your records, you’ll need to sign a release form and pay for the cost of duplication. Once your request is complete, your copies will be ready within five working days. To request a copy of the release form, contact our Medical Records department at 630-758-9719 or download a PDF of the form here. Emergency situations are expedited.
